Evidence level

Phase 3 trials

A stabilized 44-amino-acid GHRH analog approved as Egrifta for HIV-associated visceral adiposity. Among GH secretagogues, it has the strongest human evidence base — including in non-HIV populations for visceral fat and cognition.

Mechanism

Stimulates pituitary GH release via the GHRH receptor.

Researched benefits

  • Visceral fat reduction

Phase 3 trials in HIV-associated lipodystrophy (n≈800 across two trials) showed ~15–18% reduction in visceral adipose tissue (VAT) over 26 weeks vs placebo. Subcutaneous fat and limb fat largely unchanged — the effect is selectively visceral.

  • Cognitive effects in healthy aging

A 20-week trial in healthy older adults and adults with mild cognitive impairment (n=137) showed modest improvements in executive function and verbal memory associated with IGF-1 elevation. Effect size was small but statistically significant.

  • NAFLD/NASH benefit

Hepatic fat reduction and improvement in liver-fibrosis biomarkers have been demonstrated in HIV-associated NAFLD trials. Whether the effect extends to non-HIV NAFLD is being explored.

  • Lipid profile improvement

Modest reductions in triglycerides and non-HDL cholesterol accompany the visceral-fat reduction.

Risks & cautions

  • moderate

    Glucose intolerance

GH elevation is mildly diabetogenic. Fasting glucose and HbA1c can rise meaningfully in susceptible users. Monitoring is standard practice and worsening glucose tolerance is a reason to discontinue.

  • moderate

    Fluid retention and joint pain

Edema, peripheral swelling, arthralgia, and carpal-tunnel-like symptoms occur in 5–10% of users — typical GH-axis effects. Usually dose-related and reversible.

  • moderate

    IGF-1 supraphysiologic elevation

Sustained dosing often pushes IGF-1 above the upper normal range. Long-term implications for proliferative risk (colon polyps, prostate, breast) are not characterized in well-powered trials.

  • low

    Injection-site reactions

Erythema, pruritus, lipohypertrophy with repeated use of the same site. Daily dosing makes site rotation important.

Dosing

1–2 mg subcutaneously once daily, typically in the evening to align with the natural GH pulse. Most users titrate from 1 mg.

Half-life

Route

subcutaneous

Half-life is short (~30 min) but the GH pulse it triggers lasts hours. Inject on an empty stomach. Egrifta is supplied as 1 mg or 2 mg lyophilized vials with diluent. Monitor IGF-1 and fasting glucose every 8–12 weeks.

Contraindications

  • Active malignancy
  • Pituitary surgery, hypopituitarism, or hypothalamic disease
  • Pregnancy or breastfeeding
  • Active diabetic retinopathy
  • Acute critical illness (peri-surgical or trauma)

In depth

Background

Tesamorelin is a stabilized analog of human growth hormone-releasing hormone (GHRH, also called GRF-44). The native peptide is rapidly cleaved by dipeptidyl peptidase IV (DPP-IV); tesamorelin adds a trans-3-hexenoic acid group to the N-terminus that blocks DPP-IV cleavage, extending its functional half-life.

It was developed by Theratechnologies and approved in 2010 for HIV-associated lipodystrophy — a condition characterized by visceral fat accumulation in patients on antiretroviral therapy. Marketed as Egrifta (and later Egrifta SV, a reformulation with simpler reconstitution). Off-label and research interest covers visceral fat in non-HIV populations, NAFLD, and cognitive aging.

Mechanism in depth

Tesamorelin acts at the pituitary GHRH receptor, the canonical upstream control point of the GH axis:

  1. Pituitary GH release. Somatotrophs respond to GHRH with pulsatile GH release. Tesamorelin preserves pulsatility — it is not a continuous GH infusion.
  2. IGF-1 elevation. Hepatic IGF-1 synthesis rises as a downstream consequence of GH pulses. Most clinical effects (positive and negative) track IGF-1 rather than GH directly.
  3. Visceral fat lipolysis. GH and IGF-1 preferentially mobilize visceral adipose tissue. The mechanism is incompletely understood but appears to involve regional differences in lipolytic enzyme expression and β-adrenergic sensitivity.

Because tesamorelin works upstream (releasing endogenous GH) rather than substituting recombinant GH, it preserves physiologic feedback — somatostatin can still suppress the pulse, which is the basis of its better safety profile vs exogenous rhGH.

Human evidence

Among GH-axis peptides, tesamorelin has the strongest human dataset:

  • Falutz 2007 (n=412, 26 weeks) — pivotal phase 3 in HIV lipodystrophy. ~15% VAT reduction vs placebo, with comparable subcutaneous fat preservation.
  • Falutz 2008 (n=816, 52-week extension) — durability and long-term safety in HIV populations.
  • Stanley 2014 (n=50) — extended findings to hepatic fat in HIV-associated NAFLD with significant reductions in liver fat fraction.
  • Baker 2012 (n=137, 20 weeks) — first cognitive trial in healthy older adults and MCI, showing modest executive function and memory benefits.
  • Several smaller trials have explored polycystic liver disease, post-bariatric body composition, and non-HIV visceral adiposity with consistent (if modest) signals.

The HIV-population safety dataset is the largest, spanning years of real-world use under Egrifta marketing.

Preclinical evidence

The GHRH receptor pharmacology of tesamorelin is well-characterized in rodent and dog models. Visceral fat reduction translates cleanly between species. Cognitive effects in aged rats also replicate the human cognitive signal, though with the usual interpretive limits.

Stack synergy

Common stacks:

  • Tesamorelin + ipamorelin (or other GHRP) — GHRH analog + ghrelin agonist produces a larger GH pulse than either alone. Mechanistically sound; the practical question is whether the additive effect is needed or whether tesamorelin alone is sufficient.
  • Tesamorelin + metformin — sometimes used to offset the glucose-intolerance side effect. Reasonable but unstudied as a formal combination.

Avoid stacking with exogenous rhGH — bypasses the natural feedback that gives tesamorelin its safety advantage.

Open questions

  • Does long-term tesamorelin use raise cancer incidence via sustained IGF-1 elevation? The HIV cohort is informative but confounded; no clean long-term data in healthy adults exists.
  • Is the cognitive effect in MCI clinically meaningful, or a statistical signal without practical impact? Larger and longer trials would be needed.
  • What is the optimal cycling pattern for non-medical use? The approved label is continuous daily dosing; cycled regimens (e.g., 5 days on / 2 off) are common in research-peptide practice without formal data.
  • How does tesamorelin compare head-to-head against sermorelin or CJC-1295? Stabilization gives tesamorelin a kinetic advantage on paper, but no rigorous comparison exists.
